About the Role
We’re seeking a Nurse (RN or LPN) to support patients with substance use disorders (SUD) through Medication-Assisted Treatment (MAT), chronic care management, and care coordination. This role focuses on high-risk populations and works closely with our Addiction Medicine team to support long-term recovery and overall health.
This position is eligible for the Nurse Corps Loan Repayment Program

What You’ll Do
- Support MAT services and assist with care for patients with substance use disorders
- Provide triage (phone and in-person) and patient assessments
- Manage chronic care patients and coordinate follow-up care
- Educate patients on treatment plans, recovery, and overall health
- Assist providers with evaluations, treatment plans, and ongoing care
- Administer vaccines, perform point-of-care testing, and complete nurse visits
- Coordinate referrals, transitions of care, and community resources
- Participate in outreach, education programs, and team-based care initiatives
- Maintain accurate documentation and ensure HIPAA compliance
- Graduate from an accredited School of Nursing with an Associate or Bachelor of Nursing Degree
- Maintain current RN or LPN license in the Commonwealth of Pennsylvania
- MAT for Opiate Use Disorder experience would be very beneficial
- Bilingual in English and Spanish desirable but not required
- Strong clinical judgment and patient education skills
- Excellent communication and therapeutic interpersonal skills
- Organized, detail-oriented, and able to multitask
- Ability to manage multiple priorities in a fast-paced, team-based environment
- Compassionate, nonjudgmental approach to patients in recovery
