Duties and Responsibilities
Provide comprehensive nursing care to residents during the PM shift.
Assess resident conditions and collaborate with the interdisciplinary team to develop, implement, and update individualized care plans.
Administer medications, treatments, and therapies as ordered by the physician.
Monitor residents for changes in condition and promptly communicate concerns to the appropriate healthcare provider.
Supervise Certified Nursing Assistants (CNAs) and other assigned staff, providing guidance and support throughout the shift.
Delegate nursing assignments appropriately and ensure quality resident care is delivered.
Accurately document resident assessments, treatments, medication administration, and significant observations in the electronic medical record.
Educate residents and family members regarding medications, treatments, and care plans.
Serve as Charge Nurse by addressing clinical concerns, supporting staff, and coordinating resident care during the shift.
Ensure compliance with infection prevention protocols, safety procedures, and all federal, state, and facility regulations.
Collaborate with physicians, therapists, social services, dietary, and other departments to provide high-quality resident care.
Participate in shift reports, care conferences, and required in-service education.

Skills and Qualifications
Graduate of an accredited nursing program.
Current Illinois Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing.
Previous skilled nursing, rehabilitation, or long-term care experience preferred.
Current CPR certification preferred or required in accordance with facility policy.
Strong assessment, clinical judgment, and critical thinking skills.
Excellent communication, leadership, and organizational abilities.
Ability to supervise nursing staff while promoting teamwork and quality care.
Ability to work effectively with geriatric and rehabilitation populations.
Ability to perform the essential functions of the position with or without reasonable accommodation.
Must successfully complete all required background checks and pre-employment screenings.
