Full Time LVN Home Health & Hospice

Charity Home/Hospice is seeking a compassionate and experienced Licensed Vocational Nurse (LVN) to join our Home Health & Hospice team. This is an hourly position, available as Full-Time or Part-Time (minimum 20 hours per week), offering flexibility and meaningful patient care.

Responsibilities:

• Provide skilled nursing care to patients in home and hospice settings.
• Administer medications, perform wound care, and document care accurately.
• Collaborate with RNs, physicians, and the interdisciplinary care team.

Requirements:

• Current LVN license (State of [Insert State]).
• Minimum 3 years of nursing experience required.
• Experience in Home Health, Hospice, SNF, and/or Subacute settings strongly preferred.
• Valid driver’s license, reliable transportation, and current CPR/BLS certification.
