Job Summary

We are seeking an experienced Case Manager Registered Nurse (RN) to join a leading acute care healthcare facility. This is a full-time permanent opportunity for a compassionate nurse who enjoys coordinating patient care and supporting patients throughout their healthcare journey.

Position Highlights

• Full-Time Permanent Position
• Day Shift Schedule
• Monday–Friday with rotating weekends
• Pay Range: $36.96 – $44.00/hour (based on experience)
• Weekend Differential: $1.50/hour
• Sign-On Bonus: Available on a case-by-case basis

Responsibilities

• Coordinate patient care and discharge planning
• Develop individualized care plans based on patient needs
• Conduct assessments and evaluate available resources
• Educate patients, families, and caregivers regarding treatment plans and services
• Collaborate with physicians, nurses, therapists, and interdisciplinary teams
• Ensure smooth transitions of care and positive patient outcomes
• Advocate for patients while maintaining compliance with healthcare regulations

Requirements

• Active RN License
• Associate Degree in Nursing (ADN) or higher
• Minimum 2 years of recent Acute Care Case Management RN experience (within the last year)
• Strong communication, coordination, and patient advocacy skills
