RN Case Manager "High Demand"

Position Summary:
Delivers comprehensive case management services within an acute care environment, ensuring seamless coordination across the healthcare continuum. Conducts thorough patient assessments and develops, implements, and evaluates individualized care plans in partnership with a multidisciplinary team. Oversees efficient and safe discharge planning while optimizing resource utilization. Evaluates medical necessity and ensures adherence to regulatory and reimbursement standards. Acts as a key clinical resource for patients, families, and care teams, driving high-quality, cost-effective outcomes and continuity of care.

Minimum Requirements:

• Graduate of an accredited nursing program
• Minimum three (3) years of acute care nursing experience
• At least one (1) year of experience in Case Management, Discharge Planning, or Utilization Review

Preferred Qualifications:

• Three (3) years of acute care case management experience
• Experience in Pediatrics, Emergency Department, or Utilization Review
• CCM or ACM certification
