Nurse Practitioner (NP) / Physician Assistant (PA)
Location: Decatur/Springfield, IL
Schedule: Full-Time | Monday–Friday | 40 Hours/Week | No Weekends or Holidays
Compensation: $180,000 (Base + Incentives)

Join a growing mobile geriatric care team providing primary care, wellness, and chronic disease management services within assisted living and memory care communities. This role offers significant autonomy, a collaborative work environment, and the opportunity to make a meaningful impact on patient care.
Key Details
- Patient volume: 16–20 patients per day
- Mobile practice model – typically serving one community per day
- Regular visits to assisted living and memory care facilities
Requirements
- Minimum 2 years of NP or PA experience
- Active Illinois license
- DEA certification
