Position Summary
The Nurse Case Manager (RN) coordinates and manages patient care throughout the healthcare continuum using a collaborative, multidisciplinary approach. This role supports patient care planning, discharge coordination, utilization review, and care management while serving as a clinical resource for patients, families, and healthcare teams.
Key Responsibilities
- Assess, plan, coordinate, and evaluate patient care needs
- Develop and manage patient care and discharge plans
- Collaborate with physicians, nurses, and multidisciplinary teams
- Coordinate utilization review and case management activities
- Educate patients and families regarding treatment and care plans
- Ensure compliance with healthcare regulations and documentation standards
- Support continuity of care and resource management
Qualifications
- Graduate of an accredited nursing program
- Active Nevada RN license required
- BLS certification through AHA required
- Minimum 3 years of acute care nursing experience
- Minimum 1 year of Case Management, Discharge Planning, or Utilization Review experience
- Strong clinical assessment and communication skills
