Nurse Case Manager RN

Job Description:

• Case Manager/Discharge Planning – National Care Coordination Command Center
• Plans, develops, assesses, and evaluates care provided to members
• Collaborates with physicians, interdisciplinary healthcare team, and patient/family
• Reviews all inpatient admissions within 24 hours; initiates discharge planning
• Serves as liaison between inpatient and referral facilities/agencies
• Provides comprehensive case management and advocacy for patients and families

Requirements:

• Active Compact RN License
• BLS Certification (AHA)
• Previous Case Management or Discharge Planning experience required
