Job Summary
We are hiring a Licensed Bachelor Social Worker (LBSW) to assist patients with discharge planning, care coordination, resource referrals, and psychosocial support while working closely with the healthcare team.
Requirements
- Bachelor's Degree in Social Work or related Social Services field
- Active Kansas LBSW License
- Hospital-based social work experience preferred
- Excellent communication and case management skills
Pay & Benefits
- $24.77+/hour (based on experience)
