Position: Licensed Practical Nurse (LPN/LVN) – Long Term Care
Location: Kane, Pennsylvania
Shift: Nights (6:00 PM – 6:00 AM)
Schedule: 12-hour shifts | 84 hours biweekly

• 2 days on / 2 days off / 3 days on rotation
• Rotating weekends required

Compensation

• Base Pay: $26.00/hour
• Night Differential: +$0.75/hour
• Relocation Assistance: Case-by-case

Job Summary

Under the supervision of the RN Supervisor, the LPN will provide direct patient care, administer treatments, and oversee Nursing Assistants to ensure high-quality care for residents. This role is ideal for candidates passionate about working with elderly populations in a collaborative environment.

Key Responsibilities

• Administer medications, treatments, and monitor patient conditions
• Accurately document care using EMR (Physician’s Computer Company)
• Supervise and guide Nursing Assistants
• Update and maintain resident care plans
• Communicate changes in condition to RN Supervisor
• Assist physicians during rounds and procedures
• Handle admissions, transfers, and discharge documentation
• Participate in quality assurance and infection control practices
• Provide compassionate, resident-focused care

Requirements

• Active Pennsylvania LPN License (or applied at time of interview)
• BLS Certification (AHA) – Required
• New Graduates Welcome
• Long-term care or nursing home experience – preferred
• Strong communication, observation, and teamwork skills
• Knowledge of nursing practices, safety, and patient care standards

Work Environment & Expectations

• Supervise CNA team and ensure quality care delivery
• Maintain accurate documentation and compliance with regulations
• Participate in staff meetings, training, and care conferences
• Follow safety, infection control, and facility policies
