Case Manager RN

We are seeking an experienced Case Manager RN to join a leading acute care hospital in Sanford, Florida.

Schedule

• Full-Time
• Day Shift
• Monday–Friday
• Rotating weekends (every other Saturday, possibly every 3rd Saturday once fully staffed)

Compensation

• $36.96 – $44.00/hr (based on experience)
• Weekend Differential: $1.50/hr
• Sign-On Bonus: Case-by-case
• Relocation Assistance: Case-by-case

Responsibilities

• Coordinate patient care and discharge planning
• Conduct psychosocial and discharge assessments
• Develop individualized care plans
• Educate patients, families, and caregivers
• Collaborate with physicians, nurses, and interdisciplinary teams
• Ensure smooth transitions of care and resource coordination

Requirements

• Active RN License
• Associate Degree in Nursing or RN Diploma
• Minimum 2 years of recent Acute Care Case Management RN experience (within the last year)
• Strong communication and care coordination skills
