Nurse Practitioner / Physician Assistant (Home-Based Primary Care)
Location: Brooklyn/Staten Island or Bronx/Westchester, NY
Compensation: Up to $220,000 ($145,000 base + up to $75,000 performance incentive) + $10,000 retention bonus after 1 year

Job Summary:
We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive, home-based primary care to high-risk patients enrolled in the ACO REACH program. This role focuses on chronic disease management, preventive care, and reducing avoidable hospitalizations through a value-based care model.

Key Responsibilities:

• Deliver in-home primary care, assessments, diagnosis, and treatment.
• Develop personalized care plans and manage chronic and acute conditions.
• Order and interpret diagnostic tests, prescribe medications, and provide preventive care.
• Collaborate with physicians, specialists, nurses, and care managers to coordinate patient care.
• Participate in case reviews, patient panel discussions, and shared after-hours coverage.

Qualifications:

• Graduate of an accredited NP or PA program with an active New York license, Board Certification, and DEA registration.
• Minimum 3 years of clinical experience, preferably in home-based care, geriatrics, or complex chronic care.
• Proficiency with EMR/EHR systems; bilingual Spanish is a plus.
