Licensed Clinical Social Worker (LCSW) - Hybrid

About the Role:
We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to provide behavioral health services within an integrated care setting. This hybrid role focuses on delivering evidence-based mental health treatment, collaborating with interdisciplinary teams, and supporting pediatric patient populations ages 6 and above through comprehensive behavioral health care.

Compensation:

• Annual salary range: $90,000 – $95,000 based on experience

Key Responsibilities:

• Conduct patient assessments using evidence-based screening and assessment tools
• Provide behavioral health treatment utilizing CBT, Motivational Interviewing, Crisis Intervention, and Interpersonal Psychotherapy
• Collaborate with PCPs, NPs/MDs, and care management teams to develop and monitor treatment plans
• Participate in care team meetings and provide treatment recommendations
• Conduct crisis intervention services and risk assessments as needed
• Perform outreach to engage patients with limited behavioral health service utilization
• Maintain timely and accurate documentation within electronic health records (EHR)
• Attend clinical supervision sessions, team meetings, and case conferences

Qualifications:

Required:

• Active New York LCSW License required
• Master of Social Work (MSW) degree from an accredited institution
• Minimum 2 years of behavioral health or social work experience required
• Experience working with pediatric patients ages 6 and above
• Spanish bilingual speaker required
• Must reside within the Tri-State area
• Ability to work onsite once per week in Bronx, NY
• Experience utilizing behavioral health screening tools and conducting biopsychosocial assessments
• Strong understanding and application of DSM-V diagnostic criteria
• Proficiency with EHR systems and clinical documentation
• Strong communication and collaboration skills within interdisciplinary care teams
• Must be enrolled in Medicaid and Medicare as an LCSW candidate

• Hybrid position
• On-site requirement: once per week in Bronx, NY
