Case Manager RN | Full Time

We are seeking an experienced Registered Nurse Case Manager to join a collaborative healthcare team. In this role, you will coordinate patient care, facilitate discharge planning, connect patients and families with appropriate resources, and work closely with interdisciplinary teams to ensure high-quality patient outcomes.

Schedule

• Day Shift
• Monday–Friday with rotating weekends

Compensation & Incentives

• Weekend Differential: $1.50/hour
• Sign-On Bonus: Available on a case-by-case basis
• Relocation Assistance: Available on a case-by-case basis

Key Responsibilities

• Coordinate patient care and discharge planning activities
• Perform patient assessments and develop individualized care plans
• Educate patients and families on treatment plans and available community resources
• Collaborate with physicians, nurses, and multidisciplinary healthcare teams
• Promote seamless transitions of care and positive patient outcomes
• Maintain accurate documentation of case management activities and patient progress

Qualifications

• Active RN License (or APRN)
• Associate Degree in Nursing or RN Diploma required
• Minimum 2 years of recent Acute Care Case Management RN experience within the last year
• Strong communication, organizational, and care coordination skills
