Case Manager RN | Weekday Schedule | Acute Care Experience Required

Summary

Case Manager RN opportunity supporting patients and families throughout their healthcare journey in an acute care hospital setting. Requirements

• Active RN or APRN License
• Associate Degree in Nursing or RN Diploma
• Minimum 2 years of recent acute care Case Management RN experience (within the last year)
• Strong discharge planning and care coordination experience
• Experience working across multiple inpatient units

Pay

• Weekend Differential: +$1.50/hour
• Sign-On Bonus: Case-by-case
• Relocation Assistance: Case-by-case

Shift

• Full-Time Days
• Monday–Friday
• Every other Saturday (potentially every third Saturday once fully staffed)
• Shift options:
• • 8:00 AM – 4:30 PM
  • 9:00 AM – 5:30 PM
  • 10:00 AM – 6:30 PM
  • 11:00 AM – 7:30 PM
