Job Summary

We are hiring a Licensed Bachelor Social Worker (LBSW) to support patients through care coordination, discharge planning, and connecting them with community resources. This role works closely with the healthcare team to improve patient outcomes.

Compensation

• Starting at $24.77+/hour (based on experience)
• Relocation Assistance: Up to $5,000

Responsibilities

• Assess patients' social and discharge planning needs.
• Coordinate care and community resources.
• Collaborate with physicians, nurses, and case managers.
• Support patients and families throughout the care process.
• Maintain accurate documentation.

Requirements

• Bachelor's Degree in Social Work or related field.
• Active Kansas LBSW License.
• Hospital social work experience preferred.
