Nurse Practitioner - San Jose, CA

Employment Type: Full-Time
Compensation: $159,000 – $187,000 annually
Practice Setting: Community / Integrated Primary Care

Role Overview:

Seeking an experienced Nurse Practitioner to provide comprehensive, patient-centered primary care within an integrated health center model. This role emphasizes preventive care, chronic disease management, behavioral health collaboration, and community-focused care.

Key Responsibilities:

• Perform comprehensive assessments, diagnoses, and treatment planning
• Prescribe and manage medications within scope of practice
• Deliver preventive care, primary care, and ongoing health maintenance
• Conduct physical exams, review vitals, and evaluate medical histories
• Educate patients on disease processes, medications, prevention, and adherence
• Collaborate with interdisciplinary care teams, including behavioral health and wellness staff
• Refer patients to subspecialists as appropriate
• Participate in after-hours on-call rotation
• Maintain accurate, compliant clinical documentation
• Engage in team meetings, peer reviews, and quality improvement initiatives
• Assist with coverage for other clinicians as needed

Qualifications

Required:

• Master’s degree or higher as a Nurse Practitioner
• Active California NP license
• Current DEA registration
• Minimum 2 years of clinical experience
• Clinical background in Family Medicine, Internal Medicine, or Pediatrics

Preferred:

• Experience in community health or integrated care settings
• Knowledge of local, state, and federal healthcare regulations
• Strong analytical, critical-thinking, and communication skills
• Ability to work effectively within a multidisciplinary team
• Bilingual skills with one of the skills is preferred (Mandarin, Cantonese, Spanish, Tagalog, Farsi, or Vietnamese)
