RN Case Manager - Days | Up to $44/hr + Relocation Assistance

Job Summary
Join a patient-focused healthcare team as a Registered Nurse Case Manager and play a critical role in coordinating care, improving patient outcomes, and ensuring seamless transitions throughout the healthcare continuum. This position offers the opportunity to work closely with physicians, nurses, patients, and families to develop individualized care plans, facilitate discharge planning, and connect patients with the resources they need for successful recovery.
Key Responsibilities
- Coordinate patient care plans and discharge planning across all inpatient units
- Guide patients and families through treatment plans, insurance benefits, discharge criteria, and available resources
- Conduct psychosocial assessments and identify barriers to care
- Collaborate with physicians, nursing staff, and interdisciplinary teams to ensure optimal patient outcomes
- Facilitate patient and family education regarding care plans and recovery goals
- Maintain accurate documentation and communicate patient needs effectively across the care continuum
- Support safe, efficient, and patient-centered transitions of care
Qualifications
- Active Registered Nurse (RN) license or APRN license required
- Associate Degree in Nursing or RN Diploma required
- Minimum 2 years of recent acute care RN Case Management experience (within the last year) required
- Strong care coordination, discharge planning, and patient advocacy skills
Compensation & Benefits
- Pay Range: $36.96 – $44.00 per hour (based on experience)
- Weekend Differential: $1.50 per hour
- Sign-On Bonus: Available on a case-by-case basis
- Relocation Assistance: Available on a case-by-case basis

Schedule: Full-Time - Day Shift
Shift Options:
- 8:00 AM – 4:30 PM
- 9:00 AM – 5:30 PM
- 10:00 AM – 6:30 PM
- 11:00 AM – 7:30 PM
Monday – Friday *Rotating Weekends (typically every other Saturday, potentially every third Saturday when fully staffed)
