We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join an integrated healthcare team providing behavioral health services to diverse patient populations. Position Overview

The Licensed Master Social Worker will provide behavioral health assessments, evidence-based therapeutic interventions, crisis support, care coordination, and treatment planning while collaborating with an interdisciplinary care team.

Key Responsibilities

• Conduct comprehensive biopsychosocial assessments using evidence-based screening tools.
• Provide individual behavioral health treatment utilizing CBT, Motivational Interviewing, Behavioral Activation, Crisis Intervention, and Interpersonal Psychotherapy.
• Develop and implement individualized treatment plans.
• Participate in collaborative care meetings and provide treatment recommendations.
• Coordinate patient care with primary care providers, psychiatric professionals, care managers, and community resources.
• Conduct risk assessments and provide crisis intervention services as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Engage patients through outreach efforts to improve behavioral health service utilization.
• Maintain accurate and timely clinical documentation within electronic health records.
• Participate in clinical supervision, team meetings, and case conferences.

Qualifications

• Master’s Degree in Social Work (MSW) from an accredited institution.
• Active LMSW License in New York State.
• Experience providing psychotherapy and behavioral health services.
• Bilingual Spanish required.
