NP - Primary Care / Nursing Home (New Grads Welcome)

Position: Nurse Practitioner – Primary Care/ Nursing home
Location: Bronx, NY
Job Type: Full-Time
Salary: $135,000 – $150,000 (Based on experience) + Benefits

Job Summary

We are seeking a dedicated Nurse Practitioner (NP) to provide comprehensive primary care services. You will play a key role in diagnosing, treating, and managing patient health while contributing to a collaborative, patient-centered care model.

Key Responsibilities

• Perform physical exams and assess patient health status
• Diagnose and treat acute and chronic conditions
• Develop and implement care plans
• Order and interpret diagnostic tests (labs, imaging)
• Prescribe medications and treatments
• Maintain accurate patient documentation (EMR/EHR)
• Provide patient education on prevention, diet, and hygiene
• Administer vaccines and manage immunizations
• Collaborate with physicians and refer to specialists as needed

Qualifications

• Master’s Degree in Nurse Practitioner Program (Family or Adult Geriatrics)
• Active New York State NP License
• National Board Certification (AANP, ANCC, or ONCC)
• Strong clinical and communication skills
• EMR/EHR experience preferred
• Bilingual Spanish is a plus
