Job Summary:
Seeking an experienced RN Case Manager to coordinate patient care, discharge planning, resource management, and interdisciplinary communication to ensure safe, high-quality patient outcomes.
Requirements:
- Active RN license (or Compact License)
- Associate Degree in Nursing (BSN preferred)
- 2+ years of recent Acute Care experience
- ICU, CCU, Med-Surg, LTAC, or Case Management experience preferred
Compensations:
- Pay: $34.25 – $47.10/hr (based on experience)
- Sign-On Bonus: Case-by-case
- Relocation Assistance: Case-by-case
