Heritage Post Acute is seeking a compassionate and motivated Per Diem Physical Therapist (PT) to join our dedicated in-house rehabilitation team. This is an excellent opportunity to provide high-quality, patient-centered therapy services in a skilled nursing and post-acute care setting while enjoying the flexibility of a per diem schedule.

As a Physical Therapist, you will work collaboratively with an interdisciplinary team to help residents regain independence, improve mobility, and maximize their quality of life.

Responsibilities
  • Evaluate residents and develop individualized treatment plans.

  • Implement evidence-based physical therapy interventions.

  • Establish functional goals and monitor resident progress.

  • Modify treatment plans based on patient response and clinical outcomes.

  • Educate residents, families, and caregivers on treatment plans and home exercise programs.

  • Maintain accurate and timely documentation in accordance with facility, state, and federal regulations.

  • Collaborate with physicians, nursing staff, and rehabilitation team members to ensure optimal patient outcomes.

  • Participate in interdisciplinary meetings and care planning as needed.

  • Maintain compliance with all professional, ethical, and regulatory standards.

Qualifications
  • Current Physical Therapist license in the State of South Carolina.

  • Graduate of an accredited Physical Therapy program.

  • Strong clinical assessment and treatment skills.

  • Excellent communication and interpersonal abilities.

  • Experience in skilled nursing, post-acute care, or geriatric rehabilitation is preferred but not required.

  • New graduates are encouraged to apply.
