Job Summary
We are seeking an experienced RN Care Manager to coordinate patient care, discharge planning, and care transitions. You'll collaborate with physicians, nurses, and multidisciplinary teams to ensure quality, efficient, and patient-centered care.
Requirements
- Associate Degree in Nursing or Diploma in Nursing (BSN preferred)
- Active Kansas RN License
- Minimum 2 years of acute care Med-Surg nursing experience
- Strong communication and care coordination skills
Pay & Benefits
- $30.11+/hour (based on experience)
