Physical Therapist (PT) – Home Health | Poughkeepsie & Newburgh, NY | Full-Time, Part-Time & PRN

Stern at Home Therapy is hiring Full-Time, Part-Time, and PRN Physical Therapists to deliver one-on-one care to patients in their homes throughout the Poughkeepsie, Newburgh, and surrounding Hudson Valley communities.
At Stern at Home Therapy, clinicians have the autonomy to manage their own schedules while being supported by a dedicated administrative team that handles referrals, scheduling, authorizations, and logistics. Our focus is simple: provide exceptional patient care without the productivity pressure, double-booking, or clinic politics.
What You'll Do
As a Physical Therapist, you will evaluate and treat a primarily geriatric patient population in their homes, helping patients improve mobility, strength, balance, safety, and overall functional independence.
Responsibilities
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients' homes.
- Address gait training, balance, strengthening, transfers, endurance, and fall prevention.
- Recommend assistive devices, durable medical equipment, and home safety modifications.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, family members, and the interdisciplinary care team.
- Maintain compliance with all clinical and regulatory requirements.
Compensation
Full-Time W-2:
Salary Option
- $92,000–$110,000 annually
Tiered Per-Visit Option
- $55–$70 per follow-up visit
- $75 per four-unit evaluation
As your monthly caseload grows, your pay per visit increases.
Full-Time 1099
- $70–$80 per follow-up visit
- $85 per evaluation
- Annual earning potential of up to $145,000
As your monthly caseload grows, your pay per visit increases.
Part-Time W-2
- $65 per follow-up visit
- $75 per four-unit evaluation
Part-Time 1099
- $72 per follow-up visit
- $85 per evaluation
PRN
- $72 per follow-up visit
- $85 per evaluation

Qualifications
- Active Physical Therapist license in New York
- Graduation from an accredited Physical Therapy program
- Reliable transportation
- Strong communication, documentation, and time-management skills
- Ability to work independently in a home health setting
- Home health or geriatric experience is preferred but not required
