MDS Coordinator RN

Parkview Care Center, a Medicare- and Medicaid-certified skilled nursing and rehabilitation center in Fremont, Ohio, is seeking an MDS Coordinator — Registered Nurse (RN). The MDS Coordinator oversees the Minimum Data Set (MDS) assessment process for our small, home-like 40-bed community, where an MDS Nurse can truly know every resident. The MDS Coordinator ensures accurate, timely assessments that support quality care and reimbursement. Prior nursing home experience is required for the MDS Coordinator position.

MDS Coordinator Duties

• Complete and coordinate MDS assessments and care plans

• Ensure accuracy and timeliness of all MDS submissions

• Collaborate with the interdisciplinary team on resident assessments

• Monitor PDPM reimbursement accuracy

• Maintain compliance with RAI process requirements

MDS Coordinator Qualifications

• Active Registered Nurse (RN) license

• MDS experience required

• Must have experience in a skilled nursing facility (SNF) / long-term care (LTC) setting
