Director of Community Relations
Territory: Burlington, Camden, Gloucester and Salem Counties

As we continue to grow, we are seeking an experienced marketer who will serve as a key ambassador in the community—building trusted relationships with referral partners and helping families navigate the process of starting care at home.
The Role
This position blends relationship-driven business development with compassionate client engagement.
You will build strong partnerships with healthcare providers and community organizations while helping families understand their care options and feel confident in their decisions.
The ideal candidate enjoys being in the field, building long-term relationships, and serving as a trusted resource for both referral partners and families.
Key Responsibilities
Referral Development & Community Engagement
Develop and maintain strong relationships with referral partners including:
Hospitals and rehabilitation centers
Assisted living and senior living communities
Physician offices and healthcare providers
Elder law attorneys and professional advisors
Veterans organizations and community partners
Responsibilities include:
Conducting regular outreach visits and follow-ups
Identifying new partnership opportunities within the community
Representing the organization at professional events, health fairs, and educational programs
Serving as a trusted resource for partners navigating home care solutions for their clients or patients

Family Consultations & Client Intake Support
Support families during the early stages of the care journey by:
Responding promptly to referral inquiries
Assisting with scheduling initial care assessments
Conducting compassionate in-home consultations (“couch visits”) with prospective clients and families
Helping families understand services, expectations, and the onboarding process
You will collaborate closely with clinical leadership and scheduling teams to ensure a smooth transition from inquiry to service start.
Marketing & Brand Awareness
Participate in community outreach and educational initiatives
Collaborate with leadership and marketing partners on campaigns and content
Support the development and distribution of outreach materials and referral resources
Ensure messaging reflects the organization’s professional and community-focused brand
CRM Management & Reporting
Document outreach activities, referrals, and inquiries in the CRM system
Track referral activity and client conversion outcomes

Provide regular updates and insights to leadership on partnership development and growth opportunities
Performance Expectations
Maintain a consistent field presence with daily outreach to referral partners across the assigned territory
Develop and maintain strong relationships with key community partners
Participate in community or educational events each month
Track outreach and referral activity in the CRM system within 24 hours

Provide weekly outreach summaries to leadership
Qualifications
Bachelor’s degree in Marketing, Communications, Human Services, or related field preferred
5+ years experience in healthcare, home care, senior services, or community outreach
Strong relationship-building and communication skills
Ability to build trust with healthcare professionals, families, and community organizations
Experience using CRM systems and Microsoft Office
Strong organizational and follow-through skills
Additional requirements:
Reliable transportation and valid driver’s license
Ability to travel regularly throughout Burlington, Camden, Gloucester, and Salem Counties
Ability to manage multiple priorities in a fast-paced environment
Base salary: $80,000–$100,000, based on relevant experience and demonstrated business-development success, plus generous commission.
