HealthYes Talent Partners is seeking a California-licensed Nurse Practitioner or Physician Assistant for a hybrid outpatient primary care opportunity in the greater Sacramento region. This full-time or part-time position combines in-person clinic visits with telemedicine and offers a predictable schedule with no nights, weekends, or call.

Position Highlights

• $155,000–$185,000 base salary

• Collaborative clinical and administrative support

Responsibilities

• Provide primary care through in-person and virtual visits
• Evaluate and manage acute and chronic conditions
• Complete clinical documentation within established timelines
• Manage patient messages, laboratory results, prescription refills, and follow-up care
• Collaborate with physicians and interdisciplinary care teams
• Deliver culturally responsive care to a diverse patient population

Qualifications

• Active California Nurse Practitioner or Physician Assistant license
• One or more years of outpatient primary care experience preferred
• Community health or FQHC experience strongly preferred
• Epic experience highly valued
• Comfortable providing both in-person and telemedicine care
• Russian, Farsi, Pashto, Spanish, or other bilingual skills are a plus
• Strong communication, efficiency, and teamwork skills

Compensation and Benefits

• Base salary of $155,000–$185,000
