Delmar Gardens of O'Fallon has an immediate need for a Part-Time Social Worker with a focus on counseling, who is an energetic and career minded individual. Initiative and excellent interpersonal and organizational skills required. Marketing and community relations will round out this great opportunity. Bachelor of Social Work required, Master of Social Work and long-term care experience preferred.

Job duties:

• DA 124’s
• Admission assessments and agreements
• Care Plan Coordination
• Process and discharge planning
• Medicaid applications
• Grievances
• Other duties as assigned
