Join our team at Ocean Grove Post Acute, a leading provider of short-term and long-term skilled rehabilitation services, as a Social Services Director. Our facility in beautiful Millsboro, Delaware is seeking a motivated and compassionate professional to lead our social services team.
Salary Range: $65,000-75,000 per year based on experience
Job Summary
As a Social Services Director, you will collaborate with our interdisciplinary team to promote and protect the rights and well-being of our patients/residents. Your primary responsibilities will include:
• Educating staff on the role of social services in patient care
• Developing and implementing individualized care plans for each patient's social service needs
• Conducting meetings with patients/residents, families, and significant others to provide support and information
• Discharge planning activities, including family education, nursing center documentation, and referral to community agencies and suppliers
• Establishing and maintaining relationships with community-based agencies and services
Requirements
To succeed in this role, you will need:
• A Bachelor's degree from an accredited school of Social Work or related field
• At least one year of supervised social work experience in a Skilled Nursing Facility, working directly with individuals (preferred)
• Licensed in Social Work for the state of PA
