One of our clients is seeking a Psychiatric RN in Midtown NY area.CLIENT'S AVAILABLE HOURS*15-20 hours per weekSUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY* Conducts medical health assessments, behavioral health pre and post evaluations, care stabilization, and referral services to clients in need of follow up appointments * Work with the Residential Coordinator to update and maintain medication advocacy; ensuring all medication changes are documented and followed. * Using a client-centered approach to client care, the Psychiatric Nurse Practitioner will ensure that the clients receive an integrated, multidisciplinary assessment and immediate stabilization plan. * Assessments are to include client choice and/or mandated care, harm reduction, trauma-informed care, and psychosocial rehabilitation. * The Residential Psychiatric Nurse will perform with a demonstrated ability and sensitivity providing services to highly vulnerable individuals who have history of homelessness, substance affected, and/or have serious and persistent mental illness. * Refer individuals to immediate/urgent care as well as establish community connections with primary care, mental health services and substance use disorder care. * Provide preliminary health diagnosis, care, and treatment plans to clients. * Provide crisis intervention assistance when needed. * Document each client interaction, health data and notes in the AWARDS system. * Coordinate and follow-up on internal and external doctors' appointments, hospital and rehabilitation admissions of clients including healthcare assessments as needed * Collaborate with the Residential Manager, Buildings and IT Manager and other interdisciplinary staff to prioritize placement meetings for company. * Establish and maintain community health resources with direct linkages to care. * Participate in internal and external health care management meetings with Senior Director, Program Director, Case Managers, and other care coordinators when needed. * Administer medication as prescribed when needed, including but not limited to ointments, injections, ophthalmic, etc. * Manage and report quarterly on any an assessment and treatment outcomes.
