MDS Coordinator

A well-established skilled nursing facility is seeking an experienced MDS Coordinator to join its clinical team. This is an excellent opportunity to play a key role in resident care while working alongside a collaborative, supportive staff.

Responsibilities:

• Complete and oversee accurate, timely MDS assessments

• Ensure compliance with state and federal regulations

• Collaborate with interdisciplinary teams on care plans

• Conduct chart audits and maintain documentation standards

• Provide MDS guidance and support to staff

Qualifications:

• Active RN or LPN license

• 1–2+ years of MDS experience in a skilled nursing facility

• Knowledge of MDS 3.0, RAI guidelines, and regulatory requirements

• EMR experience preferred

• Strong organizational and communication skills

Compensation & Benefits:
