Hospice Social Worker and Bereavement Coordinator - Dual Salaried Position

Shalom Hospice is seeking a compassionate and skilled Hospice Social Worker and Bereavement Coordinator to join our dedicated team of healthcare professionals. Responsibilities:
Assess the emotional and psychological needs of patients and their families, and develop personalized bereavement plans to address these needs
Provide counseling and support to patients and their families, including individual and group therapy sessions
Facilitate support groups and educational programs for patients and their families
Collaborate with the interdisciplinary team to develop and implement comprehensive care plans
Educate patients and their families on end-of-life care options and advance care planning
Maintain accurate and timely documentation of patient interactions and services provided
Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
Master's degree in Social Work (MSW) from an accredited program
Licensure as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee
Ability to work independently and as part of a team
Excellent communication and interpersonal skills
Ability to maintain confidentiality and handle sensitive information
Strong organizational and time management skills
Ability to travel within a designated service area
