This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families. Key Responsibilities %CF; Manage patient care in hospice settings %CF; Coordinate with healthcare teams %CF; Provide leadership and support to nursing staff Schedule and Shift Details Schedule: Monday – Friday 8:00 am – 6:00 pm, 1 weekend per month, and 2 holidays per year Travel %CF; Local travel required covering multiple counties.

Management (1+ years)

1+ years of experience in ANY of the following:
Home Health Care
Epic Home Health
Home Health
Valid Registered Nurse (RN) in the state of Pennsylvania (PA)

American Heart Association Basic Life Support (AHA BLS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
