About the Role:

As a Medical Social Worker with Freedom at Home, you will work closely with patients and families to assess their psychosocial needs, develop individualized care plans, and provide counseling, emotional support, and education. You will also connect patients and families with community resources, financial assistance programs, and support services, and advocate for patients to ensure they receive the services and resources needed to remain safely at home.

Responsibilities:

• Conduct psychosocial assessments to identify patient and family needs

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Provide counseling, emotional support, and education to patients and caregivers

• Connect patients and families with community resources, financial assistance programs, and support services

• Assist with discharge planning and long-term care planning when appropriate

• Advocate for patients to ensure they receive the services and resources needed to remain safely at home

• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care

• Complete accurate, timely clinical documentation and participate in case conferences and quality improvement activities

Qualifications:

• Master's or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)

• Current, unrestricted Ohio Social Worker license (LSW or LISW, as applicable)

• Current CPR certification

• Minimum of one year of medical social work experience required

• Home Health experience is preferred but not required

• Valid driver's license, reliable transportation, and auto insurance
