• Provide physical therapy treatments under the direction of a licensed Physical Therapist
• Assist residents in improving mobility, strength, balance, endurance, and functional independence
• Accurately document treatments and resident progress using Net Health
• Collaborate with therapists, nursing staff, physicians, and the interdisciplinary care team
• Educate residents and caregivers on therapeutic exercises and safe mobility techniques
• Support residents in achieving individualized therapy goals and maximizing independence
• Maintain compliance with all state, federal, facility, and professional regulations

• Current Ohio Physical Therapist Assistant (PTA) license in good standing
• Skilled Nursing Facility (SNF) or Long-Term Care (LTC) experience preferred, but motivated clinicians are encouraged to apply
• Excellent communication, organizational, and teamwork skills
• Experience with Net Health is a plus
• Passion for providing high-quality, resident-centered care
