You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor patients' progress, and provide education and support to patients and families.
Responsibilities:
Provide direct patient care, including vital sign checks, wound care, and medication administration
Assist with activities of daily living (ADLs), such as bathing, dressing, and feeding
Contribute to the development and implementation of individualized care plans
Monitor and report patient progress to the healthcare team
Provide emotional support and education to patients and families
Participate in quality improvement initiatives and maintain accurate records
Collaborate with other healthcare professionals to ensure seamless care delivery
