Job Summary:

Homeside Rehab is seeking compassionate and skilled Physical Therapists (PTs) and Occupational Therapists (OTs) to join our Assisted Living Facility team in Pensacola, FL.

In this role, you will provide individualized, evidence-based therapy services to residents, helping them maintain independence, improve functional abilities, and enhance their overall quality of life.

Job Responsibilities:

• Evaluate residents and develop individualized treatment plans based on clinical assessments, functional needs, and personal goals.

• Provide evidence-based PT and OT interventions to improve mobility, strength, balance, coordination, functional independence, and overall quality of life.

• Deliver skilled therapy services to support residents in maintaining independence and achieving their functional goals.

• Maintain accurate and timely documentation of all services provided in accordance with regulatory and facility requirements.

• Collaborate with the interdisciplinary care team to ensure coordinated, effective, and resident-centered treatment.

• Communicate clearly with facility staff, residents, and caregivers regarding progress, treatment plans, and recommendations.

• Active Florida State Physical Therapy or Occupational Therapy License — required

• Basic computer proficiency for electronic documentation and clinical recordkeeping

• Strong clinical assessment, problem-solving, and interpersonal communication skills

• Ability to work independently while collaborating effectively with facility staff and the interdisciplinary care team

• Experience providing therapy services in Assisted Living, Skilled Nursing Facility (SNF), Outpatient, or Home Health settings preferred

• Minimum 1 year of clinical experience preferred

• Job Type: Full-time, Part-time, Per Diem

• Location: Pensacola, FL

• Salary: $65 – $80 per hour

• Schedule Flexibility: Choose your own hours. Day and evening shifts are available Monday through Friday, with weekend availability as needed.
