Speech-Language Pathologist (SLP/CCC)

Blossom Care Center of Cuyahoga Falls is seeking a Full-Time Speech-Language Pathologist (32 hours per week) to join our dedicated in-house rehabilitation department.

As a valued member of our interdisciplinary therapy team, you'll have the opportunity to help residents improve communication, cognition, and swallowing while making a meaningful impact every day.

• Evaluate residents with speech, language, cognitive, communication, and swallowing disorders
• Develop and implement individualized treatment plans
• Provide evidence-based therapy services to maximize functional outcomes
• Accurately document evaluations, daily treatments, progress notes, and discharge planning using Net Health and PCC
• Collaborate with physicians, nursing staff, rehabilitation professionals, and interdisciplinary team members
• Educate residents, families, and caregivers regarding treatment plans and recommendations
• Maintain compliance with all state, federal, and facility regulations

• Current Ohio Speech-Language Pathologist license
• Certificate of Clinical Competence (CCC-SLP) preferred
• Skilled nursing or long-term care experience is a plus
• Strong communication, clinical, and organizational skills
• Experience with Net Health and PCC is preferred
