We are looking for an outreach worker to meet with youth and young adults in the community to validate, inspire, and support personal goal achievement. The qualified candidate will be responsible for providing support services to children, youth, adolescents and families involved with the Department of Children and Families (DCF), The Department of Mental Health (DMH) and Children’s Behavioral Health Initiative Insurance funded services. Individual must be able to work as part of a treatment team collaborating together toward a common identified goal coupled with providing a level of direct support to families based on their identified needs. Candidate will possess a bachelor’s degree, 2 years of related experience, vehicle and valid driver’s license.

Purpose:

The Outreach Worker will provide support and help youth and young adults to feel validated, inspired, and help meet their goals.

Responsibilities Include:

  • The Outreach Worker supports, coaches, and trains the youth in connecting with existing treatment providers and in linking with new or additional resources
  • Work closely with the family and any behavioral health provider to implement the goals and objectives identified by the referring provider
  • Provides structured, goal-oriented interventions to youth
  • Support development of
    • Social Skills
    • Communication Skills
    • Daily living and self-regulation skills
  • Implement goals identified in identified treatment plans
  • Deliver services in community-based settings (home, school, community)
  • Model appropriate behaviors and reinforce skill development
  • Collaborate with CBHI service providers:
    • ICC Care Coordinators
    • IHT Clinicians
    • IHBS teams
    • Family Partners
  • Participate in team meetings and care planning processes
  • Support implementation of coordinated care plans
  • Assists the youth in communicating their needs to the treatment team when appropriate
  • Coordination with the team and/or referring provider and attend all team meetings in order to clearly outline the goals of the service and provide update on youth’s progress
  • Must ensure all services are provided in a professional manner, ensuring privacy, safety, and respecting the youth and family’s dignity, and right to choice
  • Documentation of each contact in a progress report in the provider’s service record for the youth, and share this information with the team
  • Ability to work flexible hours, including afternoons/evenings
  • Travel throughout assigned service area required
  • Participation in LUK’s On Call system

Knowledge and Experience Required:

  • Must be 21 years of age or older
  • Bachelor’s degree in Human Services field from an accredited university and 1 year of experience working with children, adolescents or transition age youth; or,
  • Associate’s degree in Human Services field from an accredited school and 1 year of experience working with target population; or,
    • Experience working with:
  • Children, youth, and families
  • Behavioral health or community-based services
  • DCF-involved families preferred
  • Familiarity with CBHI services strongly preferred
