Position Overview

Wraparound Maryland, Inc. is seeking a dedicated and culturally responsive Behavioral Health Care Coordinator to support Service Members, Veterans, and their Families (SMVFs) across Maryland’s Lower Shore region. Building on WMD’s work with the Sheila Hixson Program, this community-based initiative improves access to behavioral health support by identifying unmet needs early and removing systemic barriers to care.

The Care Coordinator provides targeted screening for mental health, substance use, and co-occurring disorders, followed by non-clinical care coordination. This role serves as a crucial bridge for SMVFs—particularly those discharging from crisis, emergency, or inpatient settings—by navigating complex service systems and addressing practical challenges like rural geography, transportation, and stigma without duplicating reimbursable clinical treatment.


Key Responsibilities & Direct Care Duties

  • Targeted Screening & Outreach: Conduct standardized screenings for mental health, substance use, and co-occurring disorders to identify unmet needs among SMVFs. Accept and process outreach referrals from regional partner organizations.
  • System Navigation & Care Coordination: Guide participants through complex behavioral health and veteran service systems, ensuring warm handoffs and timely connections to appropriate community-based care.
  • Post-Crisis Continuity of Care: Provide intensive follow-up and care coordination for SMVFs transitioning out of crisis, emergency department, or inpatient settings to prevent gaps in support.
  • Barrier Reduction: Work directly with participants to resolve practical obstacles to care, including arranging transportation, navigating rural service limitations, and addressing stigma surrounding mental health.
  • Inter-Agency Collaboration: Maintain strong working relationships with local behavioral health providers, Veterans Affairs (VA) resources, community organizations, and regional military networks across the Mid-Shore counties.
  • Documentation & Tracking: Maintain accurate, non-clinical progress notes and track participant outcomes in accordance with grant standards and organizational protocols.
