Shore Points Physical Therapy is an outpatient PT practice seeking a dedicated and licensed Physical Therapist to join our home care team on a per diem basis, providing skilled home care services throughout Monmouth and Northern Ocean County. We provide care for individuals with various orthopedic, neurological, and geriatric-related conditions. This position offers schedule flexibility while delivering high-quality, patient-centered care in a home-based setting.

Responsibilities:

• Perform comprehensive physical therapy evaluations in patients’ homes

• Develop and implement individualized treatment plans based on clinical findings

• Provide evidence-based interventions to address functional limitations, mobility deficits, and pain

• Educate patients and caregivers to promote independence and safety

• Complete timely and accurate documentation in compliance with regulatory and agency standards

• Collaborate with physicians and interdisciplinary team members to optimize patient outcomes

Qualifications:

• Current New Jersey Physical Therapy license (required)

• Strong clinical reasoning and independent decision-making skills

• Excellent communication, organization, and time management abilities

• Ability to travel throughout Monmouth and Northern Ocean County

• Home care experience preferred, but not required
