Practice Settings & Responsibilities
Acute Rehabilitation Hospital
Perform follow-up patient visits
Assist with discharge planning and coordination of care
Collaborate closely with rehabilitation teams and specialists
Skilled Nursing Facility
Provide rehabilitation-focused medical management
Conduct pain management consultations
Monitor patient progress and recovery plans
Outpatient Office
See follow-up patients discharged from acute rehab or skilled nursing facilities
Provide ongoing rehabilitation and pain management care
Coordinate continuity of care across treatment settings
Qualifications
Licensed Nurse Practitioner
Strong clinical and communication skills
Ability to work independently and collaboratively
Experience in rehabilitation medicine preferred, but not required
New graduates with strong interest in rehabilitation medicine are encouraged to apply
Compensation & Benefits
Salary: $125,000 – $140,000 annually based on experience and qualifications, plus bonus

We are seeking a full-time Nurse Practitioner to join our expanding team and provide care in acute rehabilitation, skilled nursing facilities, and outpatient settings.
