Job Title: Physical Therapist (PT)

Job Summary:
The Physical Therapist provides comprehensive evaluation and treatment services to improve patients’ mobility, strength, function, and overall quality of life. This role focuses on delivering patient-centered, holistic rehabilitation care, with an emphasis on treating adult and geriatric populations with complex medical and functional needs. The PT collaborates with an interdisciplinary care team across clinical and community-based settings.

Key Responsibilities:
  • Perform physical therapy evaluations and develop individualized treatment plans
  • Implement therapeutic interventions to improve mobility, strength, balance, and functional independence
  • Monitor and document patient progress and adjust care plans as needed
  • Collaborate with physicians, nurses, and rehabilitation staff to coordinate patient care
  • Educate patients and caregivers on exercises, mobility techniques, and fall prevention strategies
  • Provide care across outpatient, acute, inpatient, or skilled nursing settings as assigned
  • Participate in interdisciplinary care planning for complex patient populations
  • Ensure compliance with clinical standards, safety protocols, and regulatory requirements
Required Qualifications:
  • Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred)
  • Valid California Physical Therapist License or proof of license eligibility
  • Minimum of two (2) years of clinical experience preferred, particularly with frail or elderly populations
  • CPR and First Aid certification within 180 days of hire (preferred upon hire)
Preferred Experience:
  • Outpatient rehabilitation experience
  • Acute care experience
  • Inpatient rehabilitation experience
  • Skilled nursing facility (SNF) experience
  • Strong experience working with geriatric patients
Required Certification:
  • California Physical Therapist License
Ideal Candidate Profile:
  • Strong geriatric rehabilitation experience
  • Experience in outpatient or acute care settings
  • Open to interdisciplinary, holistic care environments
  • Pediatrics-only experience will not be considered
  • Strong commitment to functional, patient-centered outcomes
