Oasis Therapy Services is seeking a Physical Therapist Assistant (PTA) to provide one-on-one rehabilitation services in a skilled nursing setting in Prospect Heights, Illinois.

Responsibilities
- Provide treatments under the direction and supervision of a licensed Physical Therapist.
- Follow treatment plans addressing mobility, strength, balance, endurance, and transfers.
- Monitor residents' responses and report changes to the supervising Physical Therapist.
- Educate residents and caregivers on exercises, safe mobility, and fall prevention.
- Complete accurate Net Health documentation and collaborate with the care team.
Qualifications
- Active and current Illinois Physical Therapist Assistant license.
- Graduate of an accredited Physical Therapist Assistant program.
- Current CPR certification.
- Strong communication, organization, and documentation skills.
- Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.
