Premier Washington Rehabilitation & Nursing Center is seeking a compassionate, motivated, and dedicated Full-Time Physical Therapist (PT) to join our growing in-house rehabilitation department. If you're passionate about helping patients regain independence and improve their quality of life while working alongside a collaborative interdisciplinary team, we'd love to hear from you!
What You'll DoEvaluate residents and develop individualized treatment plans.
Provide evidence-based physical therapy services to skilled nursing and long-term care residents.
Collaborate with physicians, nursing staff, rehabilitation professionals, and interdisciplinary team members to optimize patient outcomes.
Accurately document evaluations, treatment sessions, progress notes, and discharge summaries using Net Health electronic medical records.
Participate in care conferences and communicate patient progress with families and team members.
Ensure compliance with all state, federal, and company regulations.
Current Physical Therapist (PT) license in the Commonwealth of Pennsylvania.
Graduate of an accredited Physical Therapy program.
Skilled nursing facility (SNF) or long-term care experience preferred, but new graduates are encouraged to apply.
Experience with Net Health EMR is a plus.
Strong communication, organization, and time management skills.
