The Role

We are seeking an organized, detail-oriented MDS Coordinator (LPN) to lead our Minimum Data Set (MDS) process and reimbursement accuracy.

In this vital role, you will be responsible for the timely and accurate completion of the MDS assessment process in accordance with state and federal guidelines. You will collaborate closely with interdisciplinary teams—including nursing, therapy, and social services—to ensure our residents' care plans accurately reflect their clinical needs while securing appropriate reimbursement for the care delivered.

Key Responsibilities
  • MDS Process Management: Schedule, coordinate, and complete all Minimum Data Set (MDS) assessments (OBRA and PPS) accurately and within mandated federal/state timeframes.

  • Care Planning: Oversee the development, implementation, and ongoing updates of individualized resident care plans based on MDS assessments and Care Area Assessments (CAAs).

  • Interdisciplinary Collaboration: Lead weekly IDT/MDS meetings and collaborate directly with therapy, nursing, social services, and dietary departments to ensure comprehensive care documentation.

  • Reimbursement & Compliance: Monitor clinical documentation to support accurate Medicare/PDPM and Medicaid reimbursement, ensuring audit-ready compliance with CMS regulations.

  • Data & Quality Tracking: Transmit MDS assessments electronically to state databases and monitor reports to identify clinical trend indicators or documentation gaps.

Qualifications & Requirements
  • Licensure: Active, unencumbered license as a Licensed Practical Nurse (LPN) in the State of South Carolina (or a valid compact state license) is strictly required.

  • Mandatory Experience: Prior experience as an MDS Coordinator in a Skilled Nursing Facility (SNF) or Long-Term Care setting is required.

  • Clinical & Regulatory Knowledge: In-depth knowledge of CMS regulations, RAI user manual guidelines, PDPM reimbursement models, and state survey processes.

  • Systems Proficiency: Experience with electronic health record (EHR) systems (e.g., PointClickCare, MatrixCare) and MDS transmission software.

  • Skillset: Exceptional organizational skills, high attention to detail, strong analytical capabilities, and effective communication skills.
