Position Overview

We are seeking a dedicated and compassionate Social Services Assistant to join our interdisciplinary team full-time. In this role, you will help support our residents' psychosocial, emotional, and social needs to ensure they receive holistic, high-quality care throughout their stay.

Responsibilities
  • Assist in identifying residents’ psychosocial, mental, and emotional needs.

  • Complete social histories and psychosocial assessments for new and current residents.

  • Collaborate closely with the interdisciplinary clinical team to advocate for and protect residents' rights.

  • Help maintain a safe, respectful environment by actively preventing and addressing resident concerns or grievances in accordance with facility policies and mandated reporting guidelines.

  • Assist residents and their families with transition planning, community resources, and daily care adjustments.

  • Complete required documentation accurately within the Electronic Medical Record (EMR) system.

  • Perform other related administrative and supportive duties as assigned by the Director of Social Services or Facility Administrator.

Qualifications
  • Education: High School Diploma or equivalent required.

  • Experience: Experience working with the geriatric population in a long-term care, skilled nursing, or healthcare environment strongly preferred.

  • Customer Relations: Strong customer service background with a proven track record of positive resident, family, and community relations preferred.

  • Technical Skills: Proficiency with computers and electronic medical record (EMR) systems.

  • Core Competencies:

    • Excellent communication and interpersonal skills with the ability to build rapport quickly with residents, families, and team members.

    • Strong organizational skills and the ability to work effectively both independently and as part of a collaborative team.
