PURPOSE OF THE JOB: To provide comprehensive care to patients with diabetes and/or other chronic conditions.

QUALIFICATIONS:

  1. Nurse Practitioner or Board-Certified Physician's Assistant required.
  1. Unexpired, unrestricted license to practice in any U.S. State or Territory required. Must become licensed in the State of Alaska within one year of start of BBAHC employment and maintain unrestricted Alaska license continuously thereafter.
  1. Minimum of two years’ experience as a mid-level practitioner certified BTLS/ACLS required.
  1. Experience preferred in providing diabetes self-management education/training to individuals and groups according to the current American Diabetes Association (ADA) standards.
  1. Must demonstrate knowledge of the principles of growth and development over the life span (e.g., pediatric/adult/geriatric) and assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed as described in the assigned unit's policies and procedures.

ESSENTIAL FUNCTIONS:

  1. Administers patient care based on the philosophy and objectives of the Bristol Bay Area Health Corporation and within the designated scope of practice.
  1. Participates in planning for appropriate care of patients with gestational diabetes, impaired glucose tolerance and diabetes mellitus.
  1. Provides the initial evaluation and treatment plan which includes outpatient evaluation, history taking, physical examination and patient education specific to diabetes related care.
  1. Documents observations on the chart utilizing appropriate format. Records significant normal and all deviations from normal. Formulates a problem list. Confers with physician regarding all significant deviations from normal. Completes patient care documentation within 24 hours of the patient visit.
  1. Refers complicated patient problems to the BBAHC physician for consultation or follow­ up of chronic condition.

WORKING CONDITIONS: Must be able to function effectively in a busy outpatient clinical setting. Must be able to function effectively in clinical setting with common exposure to contagious diseases. Involves periods of standing, walking, bending, and lifting. Must be able to lift up to 25 pounds, stand, walk, hear, see, and communicate effectively. Travel by small unpressurized airplanes is required and will involve occasional overnight travel in remote villages.

RELATIONSHIP WITH OTHERS: Must be able to work cooperatively with BBAHC Physicians, Community Health Aides, RNs, and other hospital staff. Must be able to work effectively in a team setting.
