Primarily weekdays during business hours. No overtime.

Position Title: Community Registered Nurse
Department: Community Living
Job Purpose: The Registered Nurse of Community Living promotes those with intellectual/developmental disabilities to live their best lives by providing assessment and coordinate medical care to persons served, offering information and opinion as a member of the PCBS Leadership Team.
Evaluated By: Director of Community Living
Supervision Given: None
Classification: Non-Exempt

Qualifications:- Current Missouri Registered Nurse license with at least two years nursing experience.
- Supervisory experience preferred.
- Ability to communicate and enforce new ideas and processes, growth-minded, solutions oriented.
- Superior oral and written communication skills in both internal and external settings
- Masterful organizational and leadership skills demonstrated previous professional success.
- Strong working knowledge of social services, consumer care, and regulations
- Must have ability to use computer and other electronic equipment.
- Must possess, maintain, and operate a personal electronic device for the purpose of communication, two-factor authentication, and tracking hours worked.
- Must have a desire to manage a team of staff supporting individuals with intellectual/ developmental disabilities and advocate for them in all possible ways.
- Must embrace a person-centered philosophy, support the principles of the CMS Home and Community Based Services (HCBS) rule, and industry trends.

- Ensures the human and legal rights of persons served are respected.
- Promotes community awareness and demonstrated involvement of the persons served.
- Provides input, opinion, and solutions as a member of the PBCS Leadership Team
- Builds an effective team by providing guidance and coaching.
- Compile and assess medical and behavioral needs on initial intake for persons served.
- Provide training to persons served promoting health and wellness (medications, first aid, nutrition, etc.)
- Ensures training compliance of assigned employees is constantly met.
Ensures Excellence in Service Provision Within Community Living
- Ensures PCBS is providing community living services in accordance with the Home and Community Based Services (HCBS) rule.
- Ensure all consumers have current physical, dental, labs and immunizations.
- Coordinate medical appointments with person served and communicate with staff family/guardian needed.
- Assist persons served with admissions to hospitals and other health care facilities.
- Ensure immediate update of case records and inform staff of medication changes.
- Ensures compliance with HIPAA regulations, including PHI and ePHI.
- Perform monthly health summary on all residential consumers.
- Verify Medication Administration Record’s (MAR’s) monthly and provide changes as needed.
- Respond to daily health concerns.
- Monitor and screen person served taking psychotropic medications for Tardive Dyskinesia
- Report all persons served and staffing related emergencies to Community Living Director
- Provide training in Medication Administration, Tardive Dyskinesia, and nutrition.
- Is an excellent team player.
- Completes tasks on time and up to the expected standard.

- Leads teammates in a collaborative way that aids improvement of skills and outcomes.
- Is a part of the PCBS Leadership Team which makes decisions about the direction of the organization.
- Attends all meetings and agency functions as required.

- Is an active member on committees and project teams for global PCBS objectives.
- Handles all other addition responsibilities as needed and required.
- Review and monitor medication errors and facilitate measures to reduce recurring incidents.
- Ensure that medical consultation forms are routed to the appropriate program.
- Provide annual medical reviews for all persons served.
- Train and/or assist staff to perform special medical services to persons served.
- Visit group homes monthly and ISL sites quarterly, or as needed.
- Serve on assigned committees (Safety, Health, and Maintenance Committee).
- Assist with the development and implementation of staff orientation and training.
- Attend and participate in ISP meetings when requested.
- Respond to after-hours emergencies whenever possible.
- Ensure compliance with HIPAA regulations in relation to persons served health information.
- Attend all meetings and agency functions as required.
- Job demands include extended periods of sitting with some periods of standing and walking.
- Will occasionally require light lifting, kneeling, stooping, crouching, or bending as appropriate.
- Will possibly require some physical intervention if confronted by a person displaying combative or potentially dangerous behavior.
- Will require periods of driving agency vehicles.
- Office based role with noise from persons served, office machines, and telephones. Will have a substantial use of computers and/or other devices and screens. May be exposed to bloodborne pathogens, infectious diseases, and parasites.
