POSITION TITLE: Activities Director

DEPARTMENT: Nursing

EMPLOYEE REPORTS TO: DON

SUPERVISES: N/A

FLSA STATUS: Non-Exempt

EMPLOYMENT STATUS: Full-time, Part-Time, Per Diem


POSITION SUMMARY

Develop activities with approval of the Director of Nursing. Coordinate all activities with other service and staff. Include individual and group activities, providing activities for ambulatory, non-ambulatory and bedfast residents.


POSITION QUALIFICATIONS

Certification in activity program from state recognized program. Desire to work with the ill, aged, and disabled. Ability to work in close cooperation with staff members. Capable of generating enthusiasm. Able to accept and utilize supervision and consultation

Requirements:

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Under supervision and review by the Director of Nursing, evaluate resident according to resident’s background, interest, abilities, physical limitations and needs for the purpose of planning a meaningful program.

· Create and implement monthly activity program.

· Provide Administration with a current schedule of planned activities.

· Promote volunteer participation from community to assist in activities for Acute unit.

· Demonstrate Colusa Medical Center philosophy on patient care.

· Support, as well as be an extension of our philosophy to rehabilitative nursing

· Develop an individual plan of care that meets the resident’s current activity interests and capabilities.

· Attend weekly patient conference.

· Orient, assist and monitor the functions of the Junior Volunteer Program.


Documentation:

· Record resident’s attendance/participation in the activity program, to include response to activities.

· Activities evaluation form completed within four (4) days of patient admission.

· Maintain past monthly activities schedule records.

· Record monthly resident council meetings.
